About the Program: Sausal Creek

Sausal Creek is a clinic-based mental health outpatient clinic funded by Alameda County, serving the Fruitvale community in Oakland. The multidisciplinary team includes licensed clinicians, nurse practitioners, and case managers who provide comprehensive support. The program focuses on case management, medication support, and brief interventions, working with individuals and community partners to create personalized recovery plans.
