  • As part of the 98É«»¨Ìà Family, Sacramento Outreach Adult Recovery (SOAR) serves 250 adults aged 18+ with serious, persistent mental health issues in an outpatient setting. Using a tiered treatment approach, SOAR supports members in the least restrictive environment in obtaining their hope, health and dreams.

  • Willow Rock is a 16-bed Psychiatric Health Facility in Alameda County for adolescents 12-17 years old who are experiencing acute psychiatric symptoms.

  • San Diego Vida ACT is a Justice Involved ACT/FSP in San Diego. This Assertive Community Treatment (ACT) Full Service Partnership (FSP) Team with Supported Housing and a Strength-Based Case Management (SBCM) component will serve 110 adults, ages 18 and over.

  • ATLAS serves adults ages 18+ with serious mental illness who have a history of lack of participation in a mental health program that have resulted in two or more hospitalizations or incarcerations within the last 36 months, or have attempted to cause harm in the last 48 months.
