Essentials At A Glance
Program Type: Community-based
To Make Referrals: Pathways to Recovery only accepts referrals for individuals currently discharging and transitioning from long-term care facilities to residence in San Diego County. A housing subsidy may be available based on client income and need. All referrals must be submitted through Optum’s Single Point of Access (SPOA) by submitting an online referral form: , or calling 1-800-842-2652 to request ACT services.

Who We Serve: Adults and transitional aged youth (TAY) residing in San Diego County with serious mental illness who have recently been discharged from county-identified long-term care facilities.
Hours of Operation:
Monday-Friday: 8:00 a.m. to 5:00 p.m.
Decision Support Center: 9:30 a.m. to 4:00 p.m.
All services are provided 24 hours a day, 7 days a week.

At Pathways to Recovery, we believe in a person’s right to full inclusion and to a meaningful life of their own choosing, free of stigma and discrimination from the moment you walk in the door. Our program provides psychosocial rehabilitative, recovery-oriented, intensive, community-based services to help members navigate the mental health system and take positive steps toward their recovery.
We believe that integrated healthcare and care coordination are key components for your recovery journey. Our job is to do whatever it takes to help you get the proper care you need and feel empowered through your choices of care. We have a full staff of peer specialists, case managers, and mental health rehabilitation specialists who are all here to help you on your path.
