Essentials At A Glance
Program Type: Community-based
To Make Referrals: Pathways to Recovery only accepts referrals for individuals currently discharging and transitioning from long-term care facilities to residence in San Diego County. A housing subsidy may be available based on client income and need. All referrals must be submitted through Optum’s Single Point of Access (SPOA) by submitting an online referral form, , or calling 1-800-842-2652 to request ACT services.

Who We Serve:
Must be a resident of San Diego County
Adults and Transition Aged Youth(TAY) with Medi-Cal
Must have a qualifying Title IX diagnosis and may also have co-occurring substance use diagnosis
A county referral for persons stepping down from a long-term care facility or a person on San Diego County’s high utilizer list
Long-term care facility diversionary referrals may be considered at the County Monitor’s discretion
Hours of Operation:
Monday-Friday: 7:00 a.m. to 7:00 p.m.
Decision Support Center: 9:30 a.m. to 4:00 p.m.
All services are provided 24 hours a day, 7 days a week.
